mRNA and next-gen vaccines in 2026: a crash course
mRNA vaccines and RNA drugs give the body genetic instructions, or silence faulty ones, instead of delivering a finished protein or small molecule.[1][2] As of October 2026 the field has its first mRNA flu vaccine, a positive phase 3 trial for a personalised mRNA cancer therapy, and US policy changes, including narrower COVID-19 vaccine labels and a court stay of federal vaccine advisory committee votes that is under appeal.[3][4][5][6][7]
Why the field matters
Most medicines are a finished molecule. An mRNA vaccine is a set of instructions instead. It tells your cells how to make one harmless protein, such as part of a virus, so the immune system learns to recognise it.[1] The mRNA does not change your DNA, and the cell breaks it down.[8] RNA drugs such as siRNAs work the other way around. They switch off the message for a harmful protein.[2]
Speed is a big reason the field matters. The first mRNA COVID-19 vaccine was authorised in just under a year. Later versions updated for new strains took as little as two to three months.[9]
RNA medicines split into two broad classes. Expression platforms (non-replicating mRNA and self-amplifying RNA) deliver an open reading frame for an antigen or therapeutic protein.[1] Silencing platforms (siRNA and antisense oligonucleotides) reduce expression of a target transcript through RISC or RNase H1.[10][11] Expression platforms are attractive because changing the antigen means changing a sequence, and in vitro transcription gives high yields.[12] CEPI notes that strain-updated COVID-19 vaccines were authorised in as little as two to three months.[9]

Key ideas
The 2023 Nobel Prize recognised the discovery that chemically modified nucleosides make synthetic mRNA usable as a vaccine.[23] Modification dampens the innate immune alarm that unmodified RNA can trigger.[24] Delivery is the second key idea. Ionizable lipids carry RNA into cells and release it from the endosome.[25] For silencing drugs, GalNAc sugars carry oligonucleotides into liver cells without a nanoparticle.[18]
Main players
Moderna and BioNTech are the leading mRNA companies. Moderna has four FDA-approved products, including the RSV vaccine mRESVIA (2024) and the COVID-19 vaccine mNEXSPIKE (2025).[3][26][27] BioNTech developed Comirnaty with Pfizer and agreed to buy CureVac; the exchange offer met its closing conditions in December 2025.[5][28] Alnylam leads RNAi. Its drug Amvuttra brought in more than $1 billion in the second quarter of 2026.[29] CSL and Arcturus market the self-amplifying vaccine Kostaive.[17] CEPI funds preparedness work, including a phase 3 trial of Moderna’s H5 bird-flu vaccine candidate.[30]
The frontier in October 2026
- Beyond COVID. The FDA approved mFLUSIVA in August 2026, after first refusing to file the application in February.[3][31] Moderna’s norovirus vaccine missed an early-success test at a phase 3 interim analysis.[32]
- Cancer. INTerpath-001 was the first positive late-stage result for an mRNA cancer treatment.[33]
- RNA drugs. RNAi has moved into heart disease. Amvuttra was approved for ATTR cardiomyopathy in 2025.[34]
- Policy. In 2025 the FDA narrowed COVID-19 vaccine labels and HHS ended 22 BARDA mRNA investments.[5][35] In March 2026 a federal court stayed the appointments and votes of the reconstituted ACIP, the federal vaccine advisory committee; the government has appealed.[6][7] On 27 August 2026 the FDA approved XFG-targeted 2026-2027 COVID-19 vaccines, keeping the narrower eligibility.[36][37] States have moved in both directions: bills to ban or limit mRNA vaccines were introduced in at least seven states in 2025, while several states changed laws in 2026 to rely less on ACIP.[38][39] See the policy debate and the tracker.

Is there an approved mRNA flu vaccine?
Yes. In August 2026 the FDA approved Moderna's mFLUSIVA for adults 50 and older, the first mRNA-based influenza vaccine. Approval for people 65 and older is accelerated approval that depends on a confirmatory trial.[3][40]
Has a personalised mRNA cancer vaccine worked in a large trial?
In August 2026 Merck and Moderna reported that intismeran autogene plus pembrolizumab met its primary endpoint in the phase 3 INTerpath-001 melanoma trial. The data were to be shared with regulators, and no approval had been announced.[4][41]
Does mRNA from a vaccine change a person's DNA?
