Medical alert systems 2026: Medical Guardian, Bay Alarm, Life Alert
Medical alert systems let an older person call a monitoring centre or emergency services at the press of a button, and many add automatic fall detection.[1][2] As of 11 October 2026, monitored plans from Medical Guardian and Bay Alarm Medical were listed from about $28 to $40 a month, while Life Alert does not post prices and reviewers report a three-year contract.[3][4][5][6] Manufacturers and surveys agree that fall detection misses some falls and can trigger by mistake.[7]
Falls are common in later life. The CDC says more than one in four older people in the US fall each year, and older adults’ falls lead to about 4.5 million emergency department visits annually.[8] Medical alert systems, also called personal emergency response systems, are one way families try to make sure help can be called. This guide compares four widely known options using published prices, manufacturer specifications and independent reviewers’ findings.
How this guide works: ContentLora does no hands-on testing. We did not buy, wear or test any of these devices. We compare what the companies publish, dated to 11 October 2026, with what independent reviewers and surveys reported, and we cite every claim. Prices change often and promotions are common, so check the company before buying. We use no affiliate links. This guide is not medical advice. Talk to a clinician about fall risk and whether a device suits you.
What these systems do
A medical alert system is a button you wear, usually as a pendant or wristband, or a smartwatch. When you press it, you are connected to a monitoring centre that can send help.[1][5] Home systems use a base station and work within a set range of it. Mobile systems have their own cellular connection and GPS, so they work away from home.[1][4] Many offer automatic fall detection for an extra fee, which calls for help if the device senses a fall.[9]
Monitored systems route calls to a staffed centre. Apple Watch Fall Detection instead dials emergency services directly and then messages the wearer’s emergency contacts with their location.[2] Key specs to compare include in-home range, backup power and battery life. Medical Guardian lists up to 1,400 feet of in-home range and a 32-hour backup battery for MG Home Cellular, and up to 5 days of battery for Guardian Flex.[1][4] NCOA’s tests measured average monitoring-centre response times of 29 seconds for Medical Guardian and 34 seconds for Bay Alarm Medical.[10]

What independent reviewers found
- NCOA (National Council on Aging) published ratings on 7 October 2026. It named Medical Guardian best overall (9.9/10) and Bay Alarm Medical best for customer service (9.8/10). NCOA discloses that it earns affiliate commissions on purchases.[10] It says it tested more than 50 systems against 70 criteria, including drop tests for fall detection and timed test calls.[12] It does not recommend Life Alert, citing higher prices, a three-year contract and no fall detection.[6]
- Consumer Reports surveyed 1,869 members in 2020, so its findings are dated.[12] It found accidental activations more common than intentional ones overall. Medical Guardian was the only brand where intentional activations outnumbered accidental ones.[7][10] About a third of fall-detection users reported a false fall alert, and fall detection usually cost extra.[7][9]
- The Senior List reported Life Alert plans starting at $49.95 a month plus activation fees, with a three-year service agreement.[6]
How to choose
- Mostly at home: a base-station system with a long button range and backup battery may be enough. Check the range against the size of the home.[1]
- Active away from home: a mobile unit or smartwatch with GPS and its own cellular connection.[4]
- High fall risk: consider fall detection, but don’t rely on it alone. Medical Guardian advises pressing the button if able, and Apple says its watch cannot detect all falls.[7]
- Already an iPhone user: Apple Watch Fall Detection has no monitoring subscription but needs a connection, and it calls emergency services rather than a care team.[2]
- Avoid long lock-ins: ask about contract length and cancellation before signing. Reviewers flag Life Alert’s three-year agreement, and Bay Alarm offers a 15-day trial.[6][3]

Questions readers ask
How much does a medical alert system cost per month?
As of 11 October 2026, Bay Alarm Medical listed plans from $27.95 to $39.95 a month and Medical Guardian from $34.95 (home) to $39.95 (Guardian Flex). Fall detection often costs extra, such as $10 a month on Medical Guardian's home system.[3][1][4][9]
Does Life Alert have fall detection?
Reviewers at NCOA and The Senior List report that Life Alert does not offer automatic fall detection and requires a three-year service agreement. Life Alert does not list prices on its website.[6][5]
Can an Apple Watch replace a medical alert system?
Apple Watch SE, Series 4 or later and Ultra models can detect a hard fall and call emergency services automatically if the wearer is immobile for about a minute, and the feature turns on automatically for users 55 and over. It calls emergency services and contacts directly rather than a monitoring centre, cannot detect all falls, and needs a cellular, Wi-Fi Calling or satellite connection.[2]
How reliable is fall detection?
Apple and Medical Guardian both say fall detection does not catch every fall. In Consumer Reports' 2020 member survey, about a third of fall-detection users said their device had registered a fall that did not happen.[7]
Which medical alert system do reviewers rate highest?
NCOA, which earns affiliate commissions, rated Medical Guardian best overall and Bay Alarm Medical best for customer service in October 2026. Consumer Reports' 2020 survey found Medical Guardian least prone to accidental activation.[10]
