Who should get meningococcal vaccines? The 2026 policy debate
In 2026 the UK and US moved in opposite directions on meningococcal vaccines. After MenB outbreaks, the UK offered a one-off MenB catch-up to new students, while US health officials briefly moved MenACWY and MenB off the universal childhood schedule.[1][2] The dispute turns on how long MenB protection lasts, what it costs and who carries the risk. A March 2026 court stay restored the earlier US schedule, and the appeal is pending.[3][4][5][6]
Meningococcal vaccines each cover only some serogroups, and MenB protection is estimated to last only one to two years.[7][3] That makes “who should get which vaccine, and when” a real policy question. This page sets out the evidence and the main positions. It gives no view on whether any individual should be vaccinated. That is a decision to make with a clinician, and nothing here is medical advice.
The facts on the table
Risk is concentrated by age. In the US, rates are highest in babies under 1, with a second peak in adolescence; 16- to 23-year-olds have the highest rates among adolescents and young adults.[8] In the UK, cases after infancy peak at ages 18-19, and first-year students face about 7 times the risk of peers who don’t go to university.[4]
Serogroups are shifting. US cases reached 463 in 2025 (preliminary), driven largely by serogroup Y, which MenACWY covers.[9][10] England’s 2026 outbreaks were MenB, which MenACWY does not cover.[11][12]
Current recommendations. CDC guidance recommends MenACWY for all 11- to 12-year-olds with a booster at 16. It leaves MenB to shared decision-making, with the preferred age 16-18.[13][14] ACIP cites an estimated 1-2 year duration of MenB protection and a high cost per QALY.[3] Before the outbreaks, the UK’s JCVI did not recommend a routine adolescent MenB booster. In June 2026 the UK began a one-off programme while the JCVI reviews the evidence.[15][1]
US policy swings. On 5 January 2026 HHS moved MenACWY and MenB to high-risk groups and shared decision-making, modelling its schedule on Denmark’s.[2][16] The AAP kept its own recommendations and opposed the change.[17] A court stay on 16 March 2026 blocked the change, and the government has appealed.[5][6]
How to read it
The UK and US disagreements are different kinds of argument.
The MenB question is about evidence and cost. Agencies on both sides of the Atlantic accept that the vaccine reduces disease: UKHSA cites a 75% reduction in vaccinated infants.[18] The objection to routine use in teenagers is that protection appears short-lived and expensive per QALY gained.[3] The UK’s 2026 compromise is to vaccinate the highest-risk cohort, new students, once, while the JCVI reviews the evidence. It reads as a bridge rather than a settled policy.[1] Antibiotics for contacts remain the main outbreak tool either way.[19]
The US January 2026 change was a different kind of decision. It was framed around aligning with countries that recommend fewer vaccines, not around new meningococcal data.[16] It also came while serogroup Y cases, which MenACWY targets, were rising.[9] The court stay has put the earlier schedule back for now, so the open question is legal authority as much as epidemiology.[5]
Pentavalent MenABCWY vaccines reduce the number of shots when both vaccines are due. A CDC model found them cost-saving in that situation but very costly as a MenACWY replacement alone. That means they ease logistics without settling the MenB policy question.[20]
What may happen next
These are possibilities with stated uncertainty, not predictions.
- UK: the JCVI’s review could make MenB for adolescents or students routine, extend the one-off offer, or let it lapse after 31 March 2027. We have no basis to say which is more likely.[1][21]
- US: the First Circuit’s ruling on the stay will decide whether the January 2026 schedule returns. The timing is unknown.[6]
- Epidemiology: US cases usually peak in January to March. The 2027 winter season will show whether the serogroup Y rise continues.[9]
Further reading
The three US meningococcal vaccine types, MenACWY, MenB and pentavalent MenABCWY, are compared in the meningitis vaccines guide.[10] Outbreak dates are in the meningitis tracker. The wider dispute over US vaccine advice, including the court case, is covered in the US mRNA vaccine policy debate.

Offer MenB more widely to young people
UKHSA says first-year students face about 7 times the risk of peers who don't go to university, and MenB caused England's fast-moving 2026 outbreaks. The UK infant programme showed a 75% reduction in MenB disease. MenACWY offers no protection against MenB.[4][22][18][12]
Target MenB by risk and cost
ACIP leaves MenB to shared decision-making because protection is estimated to last only 1-2 years and the cost per QALY is high. The JCVI did not recommend a routine adolescent MenB booster, and the UK's 2026 offer is one-off and time-limited. Outbreak control relies on antibiotics for contacts, which UKHSA says work in 90% of cases.[3][15][1][19]
Recommend fewer vaccines universally (US, January 2026)
HHS modelled its January 2026 schedule on Denmark's. It moved MenACWY and MenB to high-risk groups and shared decision-making, while keeping insurance coverage. The AAP and other medical groups opposed it, and a court stayed it in March.[16][2][17][5]

A dated, sourced timeline of meningococcal outbreaks and meningitis vaccine decisions in 2025-2026: UK MenB outbreaks, US trends and policy.
Questions readers ask
Why isn't MenB routinely recommended for all US teenagers?
ACIP recommends MenB for healthy 16- to 23-year-olds through shared clinical decision-making, citing an estimated 1-2 year duration of protection and a high cost per quality-adjusted life year.[3]
Why did the UK start offering MenB to students?
UKHSA says the one-off programme responds to the 2026 outbreaks while the JCVI reviews the evidence, and that first-year students face about 7 times the risk of peers who don't go to university.[1][4]
What did the January 2026 US schedule change do to meningococcal vaccines?
It moved MenACWY and MenB from universal recommendations to high-risk groups, with shared clinical decision-making for others. A court stay in March 2026 restored the earlier schedule.[2][5]
Are US meningococcal cases rising?
Yes. The CDC reports 463 cases in 2025 (preliminary), the second-highest since 2013, with serogroup Y driving much of the rise since 2021.[9]
